Access Requests:
Access to required Software/Applications can be obtained by completing the correct form. Forms are listed on each Facility page. To access click here, then choose your facility. Follow the directions on the bottom of the form for faxing or emailing the completed form.

Employee Educational Assistance Benefits
The Organization supports the development of employees through formal education by offering several Educational Assistance programs. Each program is designed to provide financial assistance to eligible employees who want to pursue courses of study related to hospital employment, prepare for advancement, and to develop and improve their capabilities.

On boarding a new IS&T employee
New Hire Employee Template
UMMS Information Services and Technology is committed to ensuring our new employees receive a positive on-boarding experience. The New Employee Checklist is used to guide the Supervisor through the on-boarding process. The checklist provides a list of task a Supervisorcan reference during the process.
The completed form should be signed by the New Employee and their Supervisor. It should then be placed in the employee's file.
