Request Application Access

UMMS: Application Access Requests 

Application Access for new and existing staff will now be requested online through the Service Desk Portal.

You have the ability to enter the demographic information one time! You can request access to 1 or 9 applications on one form!

These application requests are for:

1.  Access granted by UMMS IS&T supported applications

2.  Staff requiring access to an electronic system as a

• New Employee- Consultant-Student-Intern- etc.
• New access to an application for Existing Staff
• Modifying an Existing Staff in a new role

Applications that will remain with paper forms:

 Follow the existing process:

•  BWMC--Chartview/ChartScan/ChartScript or SoftMed-- Email the BWMC/HIM Department

New Access: Person needs access to an application that they currently do not have access

• Request Access for a NEW Customer
  • Customer is not listed in our records

• Request Access for an EXISTING Customer
  • Customer is listed in our records

Modify Access: Person has access to an application but they need additional rights or less rights
