Healthcare team members will be able to share their ideas throughout the life of this project.

The path toward a renewed connection with the work we love starts with the feedback from the front line. It starts with a deep understanding of all the barriers to optimizing the care you are able to provide. The initial focus is removing waste, distractions, redundancies and barriers to your ability to provide efficient and effective care to our patients.

Out of over 500 My Voice Matters Submissions, over 40% have been used to make real changes in the record.

What's Changing?

These ideas are directly from the voice of the frontline. We are listening, and we are excited to announce two new changes to the medical record.

Current Practice

Streamlined Practice

Wound and Drain Reassessment:

Some departments require q 4 hour documentation for wounds/drains

This documentation is completed for each individual drain and/or wound.

Wound and Drain Reassessment:

Add reassessment row to the top of the drain and wound groups

Allow nurses to document their reassessment was unchanged from their previous assessment or unchanged with exceptions to the previous assessment for all respective wounds and/or drains.

ED Behavioral Assessment:

System-wide EDs are utilizing the IP BH Assessment, which contains unnecessary rows that do not pertain to the ED patient.

Changed to PES Mental Status Exam:

Streamlined Exam, reducing unneeded rows for ED documentation.
