UMMS Employee Health Network

In an effort to provide all of our employees with a superior health care experience, we are making a change to our case management and transition management services for self-insured University of Maryland Medical System employees. As of April 2016, self-insured employees in need of health care support following a stay at a UM hospital will be able to take advantage of the UMMS Employee Health Network through our Population Health initiatives.

These services, previously administered by Conifer at our Midtown Campus and Active Health at the Medical Center, will provide you and your dependents as you transition from the hospital, require assistance managing chronic conditions, or wish to explore opportunities for health coaching. Similar to the services provided by Conifer and Active Health, via the UMMS Employee Health Network, employees will have access to world-class health care and sophisticated data analytics by our nurse case managers, social workers, clinical pharmacists, and health coaches. 

If you are currently an active patient or were recently discharged, an UMMS Employee Health Network contact may reach out to you to determine if you are willing to participate in these programs. Just as before, all of your medical records remain confidential and these programs are optional to you, requiring your consent before participating. Please note, if were already enrolled in a program with Conifer or Active Health, you may choose to complete the program rather than changing immediately.
