Q: What is Population Health?

Population Health provides the assets, capabilities, and programs to drive medical cost savings with improved health outcomes for patient populations that receive care from providers who are in the UMMS Quality Care Network (QCN).

Q: What is the Quality Care Network (QCN)?

The Quality Care Network, a central component of the PHSO, is the organizing vehicle for the UMMS physician base and is designed to attract and retain the best employed and independent physicians and ancillary providers in the markets served by the organization. 

The QCN is a Clinically Integrated Network that is designed within Federal Trade Commission guidelines that permit independent providers to contract collectively without violating antitrust laws.


Q: How will our care model change?

By definition, participation in the quality and care management initiatives of the CIN will require significant time and attention from physicians and their office staff. But, in return, participating physicians will be eligible to obtain financial compensation for their achievements through the clinical integration program.

Q: What is the QCN’s value proposition to providers?

The QCN enables providers to:

  • Deliver quality to current and future patients and payers, engage in collaborative group contracting with payers, and participate in performance incentives programs.

  • Access technology-enabled solutions and analytics and support chronic care management leveraging proven population health services and tools.

  • Maintain practice independence, while being part of an experienced, risk-bearing entity.

  • Promote practice transformation to a value-based model and access practice management support.

Lumeris’ ADSP® (Accountable Delivery System Platform) is a cloud-based technology platform that consolidates disparate data from across the care continuum, transforming it into actionable information for key stakeholders across UMMS.
