Digital COVID-19 Vaccine Card Now Available for MyPortfolio Users
Team members who were vaccinated at an UMMS location now have an easy way to share proof of vaccination, when needed, without having to carry their COVID-19 vaccine card. Using the MyPortfolio patient portal, individuals can access their COVID-19 vaccination record from the web or through the MyChart app (available to download on the Apple App Store or Google Play). This is a safe and easy way for team members to provide proof of vaccination directly from their MyPortfolio account by:
- Generating a QR code that can be scanned from their smartphone
- Downloading a PDF with a QR code
- Exporting their vaccination record to another application
The QR code is a secure digital version of your COVID-19 vaccine card – it contains the same information as your vaccine card (name, date of birth and vaccination information), but does not include any other details or personal health information. See below for instructions on how to access your digital COVID-19 vaccination information.
If you were vaccinated at UMMS but do not have a MyPortfolio account, you will need to create an account to access this information. To set up your account, visit myportfolio.umm.edu and click the Sign Up Now button. Once you have an account, you can access your COVID-19 vaccination information using the instructions below.
Generating a QR Code from Your Smartphone
Follow the steps below to generate a QR code with your COVID-19 vaccination information that can be scanned from your smartphone.
From the web:
- Log into your account on the web by visiting myportfolio.umm.edu.
- From the menu, go to My Record > COVID-19.
- Click on QR codes to pull up a QR code with your COVID-19 vaccination record, which can be scanned by whoever requested it.
From the MyChart app:
- Log into your account using the MyChart app.
- Once you log in, you will be greeted by a screen that says, “Share your COVID-19 info” with instructions on how to access your information from the app’s homepage.
- Click the button that says Done to proceed to the homepage.
- To access your digital vaccine card, click the QR code icon (to the left of your name). This will pull up your information.
- Click the QR Codes button to pull up your proof of vaccination.
- You can share this QR code with participating locations that require proof of vaccination (for example, restaurants, stadiums, concerts or other venues).
- To access this information from the app without logging in:
- Under Quick Access Options, you can click on the toggle key next to “View COVID-19 information on the pre-login screen.”
- Once you turn this feature on, you will not need to log into the app to access your COVID-19 vaccination information.
Downloading a PDF
Follow the steps below to download a PDF with your COVID-19 vaccination information through the web or by using the MyChart app.
- Log into your account on the web by visiting myportfolio.umm.edu or using the MyChart app.
- From the menu, go to My Record > COVID-19.
- To download your PDF:
- From the web – Select Download
- From the MyChart app – Select Download/Export and then select Download Vaccination PDF.
- Once you download your PDF, you can save it to your computer or smartphone. You can also print a copy if needed.
Exporting Your Vaccination Record to Another App
Follow the steps below to export your COVID-19 vaccination record to another app, such as a digital health wallet, directly from MyPortfolio.
- Log into your account on the web by visiting myportfolio.umm.edu or using the MyChart app.
- From the menu, go to My Record > COVID-19.
- Select Download/Export and then select Export to Health Wallet.
- If you are using the MyChart app, a series of screens will appear (see screenshots below) to:
- Verify that you have another app installed on your device into which you can export your information.
- Allow you to select which information you want to export. Select the checkbox for Vaccinations and then click Export.
- Select the app where you want to export your data and follow the instructions within that app to complete the process.

As a reminder, team members are not required to carry proof of vaccination at work with the exception of limited cases where recently vaccinated individuals (for example, newly hired team members) may be instructed to do so to confirm compliance with our COVID-19 Mandatory Vaccination Policy.
