Health Information Management (HIM) also known as Medical Records, is the practice of acquiring, analyzing and protecting digital and traditional medical information vital to providing quality patient care. It is a combination of business, science and information technology.
The HIM professional is responsible for managing the integrity of the patient medical record, coding and abstracting Inpatient, Outpatient, Ambulatory and Simple Visit coding. As well as, scanning paper documents into the EMR, reviewing provider documentation for specificity and completeness, processing patient continuance of Care and Third-Party requests for Medical Records, and performing chart correction actions for duplicate patient records and other documentation errors.
