Participating Insurance Plans for UM Medical Center, UM Rehabilitation and Orthopaedic Institute, UM Midtown Campus and Faculty Physicians, Inc.

We have prepared the Participating Insurance Plan Grid (Grid) as a quick-reference to participating insurance plans, which will allow employees to quickly determine if their facility is a Participating Facility or Provider (Par Provider) with a particular insurance company or health plan.

The Participating Insurance Plan Grid provides the:

• Insurance Plan name 
• Insurance Plan Web site  
• Participating Facility or Provider  
• Whether our facilities or providers participate with that particular Insurance Plan (Yes/No) 
• Insurance plan specifics about each plan

PLEASE BE AWARE - This Grid is only a quick reference guide. Each patient's eligibility and benefits must be confirmed prior to and at the point of service. 

Plan specifics can vary from the general details provided in the Grid. For up-to-date and accurate detailed information, always use the Plan’s web site, or call the telephone number on the patient's ID card to verify eligibility and benefits. The Web address and any available telephone number for the Plans are shown in the Grid for your use.

When accessing a Health Plan's web site, a password is not required to view general information about the Health Plan. However, a password is required to access patient eligibility and/or benefit information. To obtain a password, please follow the instructions listed on the Health Plan's web site. 

The information on the Grid is subject to change. Please verify the patient's eligibility and benefits at each encounter.
