The goal of all home-based care programs is to help each patient maintain optimal health and independence in his or her home setting, whether a family residence, a group home or a residential care facility.

UM Shore Home Care staff members work with patients, families and providers to develop an individualized program of care to meet the patient’s needs. Staff members provide education on medication, rehabilitation and safety awareness to encourage each patient's health progress. This includes tracking, reviewing and updating all care plans every 60 days or less.

Home Care Plans 

Each patient’s home care plan includes:

  • A list of needed services and their frequency
  • Equipment required
  • The professionals who provide these services
  • Timeline of anticipated results
  • Food and nutrition guidelines

All programs follow CDC safety guidelines.

Face-to-Face Encounter Certification Form

Please have your health provider fill out and submit the Face-to-Face Certification form that determines a patient meets CMS requirements for home health care.
