UM Chester River Home care helps patients maintain optimal health and independence in their home setting, which can include a family residence, a group home or a residential care facility.

We provide compassionate, professional and dependable services, including:

• Educating patients and their families to properly use equipment at home
• Ensuring compliance with medication and rehabilitation regime
• Decreasing patient hospital length of stay and readmission risk
• Increasing safety awareness and decreasing fall risks

• Monday–Friday: 8:30 am - 4:30 pm

Nursing staff is on call 24 hours a day, seven days a week.
