FY22 Applicant Communication Package

• FY22 Contract Only GME recognized programs (ACGME & Non-ACGME), whose trainees are credentialed as residents/fellows, should execute this contract
• Questionnaire For returning residents and fellows only.

Please return the questionnaire and signed 14 page contract in a single pdf to Denise DeCraene in GME at denise.decraene@umm.edu. The questionnaire should be the first page in the returned packet. The contract should be physically signed or signed with an electronic signature using DocuSign®. A typed signature will not be accepted.

All J1 contracts should be returned to Serena Roach Serenaroach@umm.edu.
