Anticoagulation Management

Our Anticoagulation Management Services provide an outpatient monitoring program for individuals who have been prescribed oral anticoagulation medications (blood thinners) such as Coumadin (warfarin). Our comprehensive approach helps decrease your risk of developing complications and works to improve your health through increased patient education and close monitoring of your medication therapy.

Referrals, Pre-registration and First Appointment

If your physician feels you would benefit from our services, he or she will need to complete and submit an Anticoagulation Request for Service form. Once submitted, you will be contacted to schedule your first appointment and to complete the pre-registration process. When you come for your appointment, please bring your photo ID, insurance cards and a list of all medications and supplements you are currently taking.
