How to Request Patient Records at UM FPI

To request copies of their medical records, patients must contact the individual departments (i.e., Family Medicine, Dermatology, ENT, Ophthalmology). Please use this Authorization to Release/Review Medical Record Information when requesting records from individual departments.

For patients who want to obtain their entire medical record, they may be able to do so by contacting the University of Maryland Medical Center's HIM Department at 410-328-6750 or by visiting UMMC's Request Medical Records page.
